As a condition precedent to submitting a grievance alleging a violation Section 1 of this Article, the Union and the employee involved shall sign and give to the Authority, on a form prepared by the Authority, a waiver of any and all rights to appeal the disciplinary action to any other forum including the Civil Service Commission. The waiver shall include a declaration that no other disciplinary review has been commenced. Section 5.
